The cemetery was called Cook County Cemetery on the grounds of Oak Forest Hospital in Oak Forest, IL.
Cook County Cemetery was the burial place for indigent people who died in Cook County and also for people who died at Oak Forest Hospital when it was a TB Sanitarium for Cook County....very confusing!!! Hope this helps. If not, I can send you the info on the death certificate.
Now it is called Oak Forest Hospital Cemetery. It is sectioned off like in 50 foot square plots, he was buried on plot 19 by a tree. We got a drawing from the security guy that gave the tour to us....they kept awesome handwritten records back then.
He was born on May 16, 1901 and died on March 28, 1937
